Patient-centered medical home
Becoming a patient-centered medical home means basing your health care delivery on the physician-patient relationship. There are a number of resources for practices wanting to become a PCMH including the AAFP-affiliate TransforMED and the Patient Centered Primary Care Collaborate, as well as a variety of resources for PCMHs already in practice.
