New Texas Health Steps child health clinical record forms now available

Physicians can now access a new set of child health clinical record forms for use in documenting completion of Texas Health Steps medical checkups. The forms can be saved on a computer as an electronic document or printed and used as paper records. Unlike the current forms, which are only available for patients up to 10 years of age, the new set includes a form for each visit under the THSteps periodicity schedule, from up to 5 days through 20 years of age.

These forms reflect current THSteps policy. A new THSteps policy will be implemented later this year and the forms will be updated accordingly. The implementation date is not known at this time, but THSteps will keep physicians informed.

Each individual form includes the required components for that age visit, along with space for documenting routine, non-required components of a medical checkup. On the back of the forms is a helpful list of suggested anticipatory guidance topics and forms such as the Parent Hearing Checklist, the TB questionnaire, and the lead questionnaire.
