The concept of the patient-centered medical home, or PCMH, continues to evolve. The Medical Group Management Association and TransforMed have gathered a team of family physicians and industry experts to help practices navigate the changes and teach them how to succeed in the PCMH model. Join MGMA and TransforMed on the “Journey to High Performance,” Nov. 11-13, 2010, in Orlando, Fla.

At the conference, attendees will be grouped into “learning levels” of basic, intermediate, or expert. Other features include:

• Collaboration with others in the same stage of transformation in tailored education sessions.
• Peer-directed learning.
• Direction from real-life PCMH practice managers who will offer lessons learned, success stories, anecdotes, and tips.
• Extra learning opportunities through informal dinner groups.
• The ability to build stronger relationships with colleagues and share the challenges faced on the journey to PCMH.
